SECTION 159.103. Applications

(a) An application must be accompanied by one completed and signed Owner/Primary Contact Background History form for the primary contact and each owner of more than 10% of the company. (b) An application may be rejected if incomplete. (c) An application is subject to no further evaluation or processing if an applicant fails to provide information or documentation within 60 days after the Board makes written request for the information or documentation. (d) License holders must retain documents establishing ownership for a period of five years from the date the application was filed.
