(a) A licensee shall inform each client of the name, address, website address, and telephone number of the department for the purpose of filing a complaint or reporting violations of the Act or this chapter on: (1) each written contract for services; and (2) a sign prominently displayed in the primary place of business. (b) A licensee or a hearing instrument fitting and dispensing practice shall provide to a client, who provides a signed, written request, a copy of the client's records that pertain to the testing for, and fitting and dispensing of, hearing instruments.
