SECTION 112.91. Change of Name, Address or Other Information

(a) A license holder or permit holder must notify the department in writing within thirty (30) days of any changes of name, address, telephone number, or employment. (b) If a license holder changes employment, the financial security on file with the department must be updated within thirty (30) days of the license holder's change in employment. The updated financial security may be filed as follows: (1) The license holder's new employer, who is subject to §112.60, must provide the license holder with a surety bond or other acceptable form of financial security. The license holder must submit the surety bond or other financial security to the department with the change in employment notice. (2) As an alternative to subsection (b)(1), the license holder's new employer, who is subject to §112.60, may submit the surety bond or other acceptable form of financial security directly to the department on behalf of itself and the license holder. The new employer must submit the prescribed form that identifies the license holder who is covered under the surety bond or other financial security.
