SECTION 26.54. Application Procedure

  • (a) The department shall issue a request for applications, to be published in the Texas Register.

    (b) The application shall be in a form prescribed by the department.

    (c) The application submitted to the department shall:

    (1) be fully and legibly completed;

    (2) be submitted in a timely manner;

    (3) be signed by an authorized individual; and

    (4) contain the following required information:

    (A) the name and address of the applying organization or entity;

    (B) the name and title of a primary contact person who may be contacted during normal business hours;

    (C) a detailed description of the proposed project, including how the project will be incorporated into the program services and a timeline of proposed activities;

    (D) the estimated number of children to be reached by the proposed activity/project if the grant is awarded;

    (E) a description of how quantifiable results will be measured if the grant is awarded; and

    (F) a proposed budget for use of grant funds, if awarded.
