SECTION 47.69. Transfers

(a) Negotiation of an individual's transfer from one provider to another. The providers involved in an individual's transfer must coordinate with the case worker to negotiate the transfer date. (b) Initiation of services. The receiving provider must initiate services on the negotiated date. The negotiated date is the begin date on HHSC's Authorization for Community Care Services form. (c) Evaluation and service delivery plan. On or before the begin date, the receiving provider must: (1) conduct an evaluation, as described in §47.45 of this chapter (relating to Pre-Initiation Activities); and (2) develop a service delivery plan, as described in §47.45 of this chapter.
