SECTION 749.303. What must I do before opening a branch office?

At least 30 days prior to the opening of a branch office, you must provide us the following information with your request to amend your license:
(1) The address, telephone numbers (if available), and office hours for the branch office; (2) The name, qualifications, and contact information of the administrative staff person who will be primarily responsible for the day-to-day operation of the branch office; (3) The name(s), qualifications, and contact information of the child placement management staff that will be responsible for child-placing activities of the branch office; (4) The name(s) and qualifications of other employees who will be involved in child-placing activities at the branch office; and (5) An updated written professional staffing plan that includes how child placement management staff, the Licensed Child-Placing Agency Administrator, and the treatment director, if applicable, will supervise services provided from the branch office.
