SECTION 137.14. Voluntary Surrender of License

(a) A license holder who does not wish to maintain a license, the legal guardian of the license holder, or other legal representative of the license holder may voluntarily surrender the license by submitting a request in writing provided that the license holder: (1) is in good standing, and (2) does not have an enforcement case pending before the board, other than cases alleging a violation of §137.5 (relating to License Holder Notification Requirements) and §137.17 (relating to Continuing Education) of this title. (b) A license that has been voluntarily surrendered may not be renewed. A license holder who has voluntarily surrendered a license may apply for a new license.
