(a) All fees paid to the department and any charges for program-related materials are non-refundable. (b) Fees will be assessed in accordance with the following fee schedule: (1) Program Provider License Fees (paid to department): (A) Initial license application and first endorsement fee--$300; (B) Additional endorsement fee--$280 per endorsement; (C) Renewal application fee--$200; (D) Reinstatement of endorsement fee--$280 per endorsement. (2) Instructor License Fees: (A) Initial license application and first endorsement fee--$50; (B) Additional endorsement fee--$45 per endorsement; (C) Renewal application fee--$40; (D) Reinstatement of endorsement fee--$45 per endorsement. (3) Instructor Training Course Fees (paid to the department's authorized representative, as directed by the department)--$425 per course; (4) Continuing Education Seminar Fees (paid to the department's authorized representative, as directed by the department)--$100 per endorsement per seminar; and (5) Fees for Program Course Materials must be paid to the department's authorized representative, as directed by the department. (c) A duplicate/replacement fee for a license issued under this chapter is $25. (d) A dishonored/returned check or payment fee is the fee prescribed under §60.82. (e) The fee for a criminal history evaluation letter is the fee prescribed under §60.42. (f) Late renewal fees for licenses issued under this chapter are provided under §60.83.
