SECTION 116.101. Changes of Name or Address

(a) The licensee shall notify the department of changes in name or mailing address within thirty (30) days of such change(s) on a department-approved form or using a department-approved method. (b) Notification of name changes must be submitted to the department and shall include a copy of a marriage certificate, court decree evidencing such change, or a social security card reflecting the new name. (c) To receive a duplicate license, the licensee shall submit the duplicate/replacement fee required under §116.110.
