(a) Fees paid to the department are nonrefundable. (b) Hearing Instrument Fitter and Dispenser License: (1) Initial application fee (includes two-year initial license)--$205; (2) Renewal application fee (for two-year license)--$205; (c) Apprentice Permit: (1) Initial application fee (includes one-year permit)--$205; (2) Extension fee (one-year term, may only be extended once)--$25; (d) Temporary Training Permit: (1) Initial application fee (includes one-year permit)--$205; (2) Extension fee (one-year term, may only be extended once)--$25; (e) The fees for the written test, the practical test, and the jurisprudence test are set by and payable to the department's designees. (f) Continuing education provider fee--$200 annually. (g) A duplicate/replacement fee for a license or permit issued under this chapter is $25. (h) Late renewal fees for licenses and permits issued under this chapter are provided under §60.83 of this title (relating to Late Renewal Fees). (i) A dishonored/returned check or payment fee is the fee prescribed under §60.82 of this title (relating to Dishonored Payment Device). (j) The fee for a criminal history evaluation letter is the fee prescribed under §60.42 of this title (relating to Criminal History Evaluation Letters).
