An application for variance shall include:
(1) the name, address, and signature of the elected or appointed official(s) having jurisdiction of the facility for which the variance is being requested; (2) the specific statute, sections of statutes, and/or rules for which the variance is being requested; (3) the existing condition(s) that requires a variance or the desired condition(s) that would require a variance; (4) the projected cost to achieve compliance without a variance; (5) the effect the variance, if granted, would have on the operations and sanitation of the facility and on the custody, safety, health, and supervision of the inmates; (6) the length of time for which the variance is being requested; (7) any additional information or attachments demonstrating justification for the variance; and (8) utilization of alternatives to incarceration, including diversion initiatives and reentry efforts to reduce recidivism.
