(a) Upon intake, a file on each inmate shall be established. The file shall include: (1) name of inmate with aliases; (2) description; (3) gender (4) marital status; (5) address; (6) date of birth; (7) offense charged; (8) date of commitment; (9) previous criminal record; (10) record of injuries; (11) inmate property inventory; (12) disabilities warranting special accessibility consideration; (13) name, address, and phone number of person to be contacted in event of emergency; (14) the name of the delivering officer, and the arresting agency; (15) documents that purport to legally authorize the inmate's commitment. (b) Upon intake, a medical record shall be established and shall be kept separate.
