(a) In addition to the requirements of Texas Occupations Code, §169.004, for purposes of identification the physician's registration must include the patient's address, the last four digits of the patient's Social Security number, and if applicable, the name of the patient's legal guardian. (b) Physicians registered as prescribers of low-THC cannabis under Texas Occupations Code, Chapter 169 must immediately inform the department of any change to their qualifications to prescribe under §169.002.
