SECTION 251.218. Correspondence  


Latest version.
  • All correspondence with the State Office of Risk Management concerning a claim must contain the following information:

    (1) the name of the injured employee;

    (2) the social security number of the injured employee;

    (3) the date of injury;

    (4) the name of the employing agency;

    (5) the director's claim number and the commission number, if known.
