SECTION 749.3523. What specific information must I obtain from birth parents that voluntarily relinquish their parental rights?

A parent who signs an affidavit of voluntary relinquishment of parental rights regarding a biological child must also prepare a medical history report form that we issue as required by §161.1031 of the Texas Family Code. If the child is:
(1) In the managing conservatorship of Child Protective Services, DFPS must maintain the form and make it available to persons with whom the child is placed; and (2) Placed for private adoption through a licensed child-placing agency, that agency must maintain the form.
