SECTION 745.9119. How do I demonstrate my criteria for revoking or denying a credential or otherwise reprimanding a credential holder?

To demonstrate your criteria for revoking or denying a credential or otherwise reprimanding a credential holder, you must provide us with the following:
(1) A clear statement of the basis for revocation or denial of the credential; (2) A clear statement of the basis for a reprimand and the resulting consequences; (3) A list of any other negative sanctions, basis for the sanctions, the resulting consequences, and procedures for removal of the sanction; (4) A plan for notifying us when a negative sanction is imposed; and (5) A clear statement of the credential holder's right to review and procedures for requesting this from you.
