SECTION 745.9109. What must I submit to Licensing to demonstrate how I will ensure the continuity of my credential program?

To demonstrate how you will ensure the continuity of your credential program, you must provide the following:
(1) A statement of your mission or purpose, your number of years in existence, and a description of your target audience; (2) A description of the organizational structure of your credential program and your experience or track record in related fields; (3) A statement of training outcomes and competencies participants will attain from the training; and (4) A statement of your plan for communicating with us, credential candidates, and credential holders, including your system for maintaining a database or keeping records of the following for at least three years: (A) Candidate applications for the credential program; (B) Names and social security numbers of persons granted a credential; (C) Effective dates and renewal dates for persons granted a credential; and (D) Adverse actions against persons granted a credential.
