(a) A center must designate an IDT. (b) The IDT must monitor the services provided to the minor at the center. (c) A center must designate an RN to be a member of the IDT to: (1) provide coordination of care for the minor; (2) ensure continuous assessment of the minor's and the minor's parent's needs; and (3) implement the minor's interdisciplinary plan of care. (d) The IDT must prepare a written plan of care for the minor as described in §15.607 of this division (relating to Initial and Updated Plan of Care). (e) The IDT must include: (1) the minor's prescribing physician; (2) the center's nursing director or an RN designated by the nursing director; (3) the minor; (4) the minor's parent; (5) a social worker, if the minor is receiving social services at the center; and (6) another individual providing basic services to a minor if the minor is receiving basic services other than nursing services at the center. (f) The IDT must participate in the development of a plan of care with goals and objectives for a minor that includes discharge planning when goals and objectives are met.
