(a) The MI specialized services team must include: (1) the resident with MI; (2) the resident with MI's LAR, if any; (3) the QMHP-CS assigned to the resident with MI; (4) a representative of the LMHA or LBHA providing the MI specialized services; (5) the MCO service coordinator, if the resident with MI does not object; (6) a NF staff person familiar with the resident with MI's needs; and (7) if the resident with MI is transitioning to the community: (A) a representative from the community program provider, if one has been selected; and (B) a relocation specialist. (b) The MI specialized services team may also include a concerned individual whose inclusion is requested by the resident with MI or the LAR.
