SECTION 275.67. Termination of Services

A provider may terminate services for the same reasons for suspending services, listed in §44.308 of this subchapter (relating to Suspension of Services). If a provider intends to terminate services the provider must:
(1) send written notice of the termination to the individual; (2) inform the individual in the written notice of: (A) the termination date; (B) the reason for termination; (C) the right to appeal the termination decision in accordance with §44.503 of this chapter (relating to Fair Hearing); and (D) the individual's right to continue to receive services pending a fair hearing; and (3) notify the DADS regional designee of the termination in writing.
