SECTION 27.15. Provider Qualifications

A provider shall not be approved as a provider of Case Management for Children and Pregnant Women unless they meet the following qualifications:
(1) completion of application process and approval by the department; (2) agreeing to comply with the rules, policies, and procedures of the department relating to Case Management for Children and Pregnant Women; (3) agreeing to comply with applicable state and federal laws governing participation of providers in the Medicaid program and to enroll as a state Medicaid provider; (4) be a provider who meets, or employs or contracts with, one or more case managers who each meet, the qualifications specified in §27.21 of this title (relating to Case Manager Qualifications); and (5) has never been terminated by the department and is not listed on the HHSC Office of Inspector General's Excluded Individual Listing.
