SECTION 186.9. Reissuance of Certificate Following Revocation

(a) The applicant must complete in every detail the application for reissuance of a certificate following revocation including payment of the required application fee. (b) The applicant must appear before the advisory board to state the reasons for the request for reissuance of certificate. (c) Application for reissuance of a certificate following revocation cannot be considered more often than annually. (d) Reissuance of a certificate following revocation shall be at the discretion of the advisory board upon a showing by the applicant that reissuance is in the best interest of the public. (e) A person may not apply for reissuance of a certificate that was revoked before the first anniversary date on which the revocation became effective.
