The secretary of state will provide forms for use in filings when required by law, or when convenient for the Office of the Secretary of State. Forms are available on the secretary of state web site at: http://www.sos.state.tx.us/corp/forms_boc.shtml and http://www.sos.state.tx.us/corp/forms_reports.shtml.
