(a) A statement of officer form containing the language required by Article XVI, §1 of the Texas Constitution is available on the secretary of state web site at http://www.sos.state.tx.us/statdoc/statforms.shtml or may be obtained by writing the Statutory Documents Section, Office of the Secretary of State, P.O. Box 13550, Austin, Texas 78711-3550. See Form 2201. (b) A statement of officer must provide: (1) the language required by Article XVI, §1(b) of the Texas Constitution; (2) the specific office to which the officer has been elected or appointed; and (3) the typed or printed name and signature of the officer. (c) In addition to other accepted methods of delivery, the statement of officer may be submitted to the secretary of state by facsimile.
