To be eligible for the Former Foster Care Children's Program, a person must:
(1) have been under the conservatorship of this State upon attaining age 18; (2) have received Medicaid at the time he or she left foster care; and (3) be 18 to 25 years of age (coverage continues through the month of his or her 26th birthday).
