SECTION 354.1004. Retention of Records

The provider must maintain all records necessary to fully disclose the services provided. These records must be retained for a period of five years from the date of the service, or until all audit questions are resolved, whichever is longer. Records and supporting information must be made available upon request, regarding any payment of claims for services or supplies by the provider, to the Texas Department of Health or its designated agent.
