SECTION 351.19. Requesting a Correction of Personal Information

(a) A request to correct information collected about an individual must be in writing and must: (1) identify the individual requesting the correction, (2) identify the information to be corrected, (3) state why the information is incorrect, (4) include any evidence that shows the information is incorrect, (5) state what correction is requested; and (6) include a return address, telephone number or email address at which the individual can be contacted. (b) If the Health and Human Services Commission (HHSC) cannot identify the individual, locate the information in question, or determine what correction is requested, HHSC may contact the individual to clarify the request.
