SECTION 60.30. Initial License Applications

(a) All license applications must be submitted on Department-approved forms. (b) An applicant must complete all licensure requirements within one year of the date the application is received by the Department, or the application shall be deemed void. (c) If the applicant does not meet the deadline established in subsection (b), the applicant must reapply for a new license by complying with the requirements and procedures, including any examination requirements and payment of fees.
