(a) All podiatric medical offices shall contain the minimum amount of treatment equipment and facilities so that the podiatric physician may provide his usual and customary podiatric medical services. (b) The office must be adequately staffed and equipped to ensure that treatment is timely and properly administered. (c) It shall be the responsibility of each practitioner to ensure that any change of address or phone number(s) for each practitioner's location(s) are reported to the department no later than ten (10) business days after the change is made.
