(a) The department will issue an applicant, whose application has been approved, a license containing the licensee's name, license number, and expiration date. (b) An assistant in speech-language pathology license issued by the department remains the property of the department. (c) The department will issue a duplicate license upon written request using a department-approved form or in a manner prescribed by the department and payment of the duplicate/replacement fee under §111.160.
